Dear Parents/Guardians, Do you have a student in RSU #57 entering grade 6-12 for the 2016-17 school year? If your student is planning on participating in school-sponsored sports programs RSU #57 would like to remind you that students playing sports are required to show proof of having a physical exam within 2 years of the planned sports season. RSU #57 Health Services will be offering free Sports Physicals on Monday May 16 from 4:00PM to 6:00PM at the Massabesic High School West building School Nurse office. These physicals are for the purpose of meeting pre-participation requirements for school-based sports programs. Students who will be entering grades 6-12 and who are uninsured or underinsured may use this service. The free sports exams will be given by the RSU 57 district physician, Dr Craig Wallingford. All students must come with their parent/guardian the day of the physicals. In order to schedule an exam, a parent/guardian must call Nancy Sobanik, RN, Massabesic High School Nurse at 247-3141 (Ext 2215) during the hours between7AM and 2:30 PM on school days. All physicals must be pre-registered as spaces are limited. Physical
exam pre-participation forms will need to be filled out by a parent
before the exam. These forms will be available the day of the exam, but
to save time can be obtained at school from the nurse or downloaded
